Transitions Is The Caring AlternativeTransitions is a volunteer-based palliative care program designed to provide support for residents of the community with a serious illness. Prospective patients meet with a case manager who can assess patient needs and bring in community resources. Specially trained, experienced and caring volunteers offer support, friendship and respite to patients and their families while they live with an illness. The case manager will follow-up periodically to see if any needs have changed.
Transitions Volunteers Provide:- Socialization/Companionship
- Emotional Support
- Assistance with errands
- Respite break for the family
Transitions Provides Case Management Services:- Identification of client and family needs
- Referrals and links to community resources in order to fulfill those needs
